You may download and print the form listed below, for transferring medical records before coming to our office. You may either bring them with you to your visit or fax them to us before your appointment. Please be sure to bring all current insurance information that you may have for your child, as well as any previous medical records and shot records that you may be able to obtain prior to your initial visit at our office.
